Coordinating Better Care
Recognizing the many complexities in managing diabetes, DAN set a goal to coordinate the best system of self-management education and support for those with diabetes. Diabetes education that leads to informed self-management decisions and actions is key to reducing emergency room visits and hospital admissions and ultimately improving one’s quality of life. Many who have diabetes have not participated in a diabetes self-management education (DSME) program that focuses on the individual’s unique needs. DAN built its’ clinic to community interactive group education program based on the American Diabetes Association approved Life With Diabetes curriculum.
Diabetes Self Management Education (DSME)
DAN’s DSME has gained official recognition status for meeting the National Standards for Diabetes Self-Management Education and Support from the American Diabetes Association. Clinical professionals will deliver the education sessions under the oversight of a Quality Coordinator. The interactive sessions will be tailored by type of diabetes and age of the participants. The program will consist of nine hours of interactive group sessions and one hour of an individualized session with a Registered Dietitian.

Founders & Affiliates
The Diabetes Alliance Network (DAN) was established in 2018 by the Help A Diabetic Child Foundation, a 501c3 non-profit whose mission supports funding insulin, medical supplies and services that benefit children with type 1 diabetes. DAN has now expanded to serve those with type 1, type 2, pre-diabetes and developed a clinic to community diabetes self-management education and support (DSMES) program.
Establishing DAN was a grassroots effort of philanthropists, people affected by diabetes, physicians and other healthcare providers. DAN was designed to improve communication between agencies that directly, or indirectly serve those with diabetes to address barriers that people with diabetes may experience. Together, agencies partner with DAN to collectively bridge gaps in resources, care and diabetes education.
Although formed as a Florida non-profit, the goal of DAN is to expand the model of DSMES and duplicate services, as a best practice for linking the clinic to the community nationwide.
